Team Support Staff

Department: Care Coordination
Location: Omaha, NE

Monday - Friday, no weekends or holidays.

JOB SUMMARY:

Responsible for the administrative duties associated with triaging patients, including contacting patients to obtain information, referral coordination, and communication with primary care team to ensure tasks are completed.

ESSENTIAL JOB FUNCTIONS:

  • Coordinate the registration of new patients by obtaining necessary information, including past medical records, abstractions and demographics.
  • Coordinate patient referrals to specialists and collect/abstract referral records when visit is completed.
  • Identify and reconcile gaps of care for patients, including quality measures that have not been met.
  • Ensure all necessary documents are properly saved in patients’ electronic health record.

KNOWLEDGE, SKILLS & ABILITIES:

  • Knowledge of patient care standards and practices.
  • Knowledge of medical terminology.
  • Knowledge of HIPAA privacy rules.
  • Skill in communicating in a professional manner, both verbally and in writing.
  • Skill in planning, organizing, and time management.
  • Skill in using a computer and a variety of software, including Electronic Health Records (EHR) software, Word, Excel, and Outlook.
  • Skill in managing multiple priorities and problem solving.
  • Ability to work effectively under pressure to meet established goals and objectives.
  • Ability to maintain high levels of professional and personal integrity.
  • Ability to convey professional demeanor and sound judgment in dealing with patients’ care, protection of patient privacy, and confidentiality.
  • Ability to work independently and in a team environment.
  • Ability to act as a good representative of the company.

EDUCATION & EXPERIENCE:

High School diploma or GED required. A minimum of one (1) year of experience in the medical field utilizing Electronic Health Records (EHR) required. Additional one (1) year of experience in a clinical setting, Family Practice or Internal Medicine preferred.

WORKING CONDITIONS AND PHYSICAL EFFORT:

This role operates in a healthcare setting. This position requires frequent standing, walking, and computer work. This position allows employee to moderately vary physical position or activity for comfort.

  • Must be able to:
  • Sit 75% or longer of an 8-hour workday.
  • Walk 15% or longer of an 8-hour workday.
  • Stand 10% of an 8-hour workday.
  • Requires ability to lift and maneuver up to 10 pounds occasionally.
  • Requires repetitive use of hands for: simple grasping, fine manipulation, computer use.
  • Requires all sensory skills (speech, vision, smell, touch, and hearing) corrected to near normal range.
