Clinical Triage Nurse - Part Time

Monday - Friday, no weekends or holidays

3 days a week, 8 AM - 4:30 PM

JOB SUMMARY

The Clinical Triage Nurse is the primary contact for all patients and are the voice for the provider’s entire clinical team. This role works with a multidisciplinary team to ensure that all inbound and outbound telephone calls are triaged in a manner such that appropriate levels of healthcare resources are deployed to each patient in a friendly and courteous manner.

ESSENTIAL JOB FUNCTIONS

Speak on telephone with patients to assess symptoms, provide education and recommendations, and determine urgency of care needed.

Collaborate with physicians to determine what tests or services need to be ordered before the patient is seen by their primary care physician for both routine and acute visits, e.g., labs, imaging, clinical pharmacist, and behaviorist.

Responsible for reviewing outside clinical data, e.g., labs, x-rays, and consultation reports and delegating to the proper team member.

Coordinate patient specific care with nursing homes, home healthcare agencies, and hospitals.

Provide education related to test results, lab work, new medications and treatment plan.

Make outgoing calls to patients to schedule follow-up appointments, labs, and imaging tests.

Other work flow tasks, such as handling outside diagnostic results, tasking orders requiring provider signatures, triaging health record requests, and answering patient Web Portal questions.

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of scope of practice and patient care standards/practices.

Knowledge of proper triage protocols and procedures.

Knowledge of medical terminology.

Knowledge of HIPAA privacy rules.

Skill in communicating in a professional manner, both verbally and in writing.

Skill in planning, organizing, and time management.

Skill in using a computer and a variety of software, including Electronic Health Records (EHR) software, Word, Excel, Access, and Outlook.

Skill in managing multiple priorities and problem solving.

Ability to work effectively under pressure to meet established goals and objectives.

Ability to maintain high levels of professional and personal integrity.

Ability to convey professional demeanor and sound judgment in dealing with patients’ care, protection of patient privacy, and confidentiality.

Ability to work independently and in a team environment.

Ability to act as a good representative of the company.

EDUCATION & EXPERIENCE

Active certification as a Registered Nurse (RN) required. A minimum of two (2) years of experience as an RN utilizing electronic Health Records (EHR) and one (1) year of experience in clinical triage required.

WORKING CONDITIONS AND PHYSICAL EFFORT

• This role operates in a healthcare setting. This position requires frequent sitting, and computer work. This position allows employee to moderately vary physical position or activity for comfort.

• Must be able to:

Stand 5% or longer of a workday.

Walk 10% or longer of a workday.

Sit 85% of a workday.

• Requires ability to lift and maneuver up to 10 pounds occasionally.

• Requires employee to: bend, squat, kneel, and reach above shoulder level occasionally and twist occasionally.

• Requires repetitive use of hands for: simple grasping, fine manipulation, computer use.

• Requires all sensory skills (speech, vision, smell, touch, and hearing) corrected to near normal range.
